OFFICE FINANCIAL POLICY
(Insurance Information)
PAYMENT IS EXPECTED AT THE TIME SERVICE IS RENDERED.
We offer both VISA and MASTERCARD for your convenience. If you have dental
insurance, we are glad to file this for you. However, you will be
required to pay your co-payment (your percentage not covered by the company)
at the time of service. In cases of extended or complex therapy, payment
schedules will be defined prior to start of treatment. Our front office
will gladly assist you in these matters, and answer any questions pertaining
to your specific needs. You may simply upon request, see our current fee
schedule. Further information regarding insurance statements, account
balance or our credit policy can be obtained by calling our business office
Monday through Friday from 8:30 AM to 4:30 PM.
PLEASE NOTE: Extended payment plans must be first discussed
with Dr. Burcham. Your insurance policy information should be predetermined
